Left Atrial Myxoma Mimicking Thrombus


Abstract
The author experienced recently a thrombus-like myxoma in left atrium. A 68 year old female pati-nt
has admitted complaining of progressive exertional dyspnea since 2 years prior to this visit.
On admission, electrocardiogram revealed atrial fibrillation with frequent VPC and rapid ventricular conduction; and echo-dense round mass was detected in left atrium on two dimensional echocardiogr. m. The mass demohstrated horse shoe shaped attatchment to postern-superior margin of fossa ovalis with no stalk-or peduncle.
On gross examination, an ovoid rubbery solid mass, 5.8 x 3.8 x 3.0cm, had greyish-brown glisteni g surface. On section, it showed diffuse red-brown area in the greater part with evidence of hemorrhage and scattered gelatinous and yellow calcific areas were observed in it.
On microscopic examination, typical myxomatous stroma composed of eosinophilic matrix was fou d along with uniform, small and polygonal cells. Moreover, foci of lymphocytes, hemorrhage, granule of hemosiderin, elastic fibers, reticular fibers and calcium were also observed in the stroma.
Following the operation, she recovered well showing uneventful course and was discharged in fine c.ndition on the 18th postoperative day.
